Care & Services at The Terrace Assisted Living community are designed to meet the customized needs of each individual.
The Terrace Assisted Living community offers a wide range of assisted living and rehabilitation services in our comfortable and caring environment. With the full attention of our highly trained and certified staff members, our community of residents receive state-of-the-art care.
Prior to move-in, each applicant receives a care needs assessment with our Admissions Liaison to determine the appropriate Care Level required (Care Levels are shown below). Each resident is then periodically evaluated to address any changes that occur. Following a universal care model, the staff at The Terrace Assisted Living community works to promote each resident’s highest quality of life – social well-being, wellness, and independence. Should the need arise, we can provide comprehensive, professional skilled nursing and rehabilitative care as well as coordination of physician and pharmacy services.
And our Care & Services don’t stop there. Should a natural or man-made disaster occur, The Terrace Assisted Living has a complete Emergency Management Plan that has been developed and implemented to protect each resident from harm and to move every individual to a safe location along with his/her identification, medications, and care plan.
Basic
The Terrace Assisted Living offers each resident the following Basic Care:
- Secure/private apartment with kitchenette
- Full bath with easy step-in shower
- Personal climate control
- Three meals per day served on china in our comfortable dining area
- Snacks and beverages all day
- Daily laundry and housekeeping
- All utilities, including phone and basic cable television
- Expanded basic cable and telephone available
Intermediate
The Terrace Assisted Living Intermediate Care Level offers the following services in addition to the Basic Assisted Living Care:
- Shower/bathing assistance
- Activities of daily living assistance
- Oxygen concentration and/or blood glucose monitoring
Intermediate Plus
The Terrace Assisted Living Intermediate Plus Care Level offers the following services in addition to the Basic and Intermediate Assisted Living Care:
- Cues for safety during transfers to/from wheelchair
- Showering and activities of daily living management
- Incontinence management
- Behaviors or wandering monitoring
- C-pap or nebulizer breathing treatments management
Intermediate Extensive
The Terrace Assisted Living Intermediate Extensive Care Level offers the following services in addition to the Basic, Intermediate, and Intermediate Plus Care Levels:
- Feeding by staff
- Mechanical lift or 2-person transfer from bed to chair
- Extensive or total showering/bathing care
- Extensive monitoring of complex medical conditions (including gastrointestinal tube feeding, chest tube, and wound vac)
- Frequent monitoring for aggressive behaviors toward others or flight (exit) behaviors
- Hospice or end-of-life care (including frequent assessment to determine if the care needed exceeds the licensing of The Terrace)
At this level of care, most residents are declining and may be required to move to the Skilled Nursing Facility level of care. Each case will be reviewed with the resident and family to ensure the best possible outcome.
Families and the resident also have the right and option to provide – at their cost – additional one-on-one staffing through an outside staffing agency. If the added services allow for the resident’s needs to be met, and if The Terrace can create a care plan that ensures the resident’s needs – as well as all regulations – are met, the resident, at the discretion of The Terrace Assisted Living, may remain at The Terrace.
